LOST DUTCHMANSPORTING CLAYS

WAIVER OF LIABILITY RELEASE, HOLD HARMLESS AND INDEMNIFICATION AGREEMENT. I wish to engage in firearms shooting at Lost Dutchman Sporting Clays, LLC (LDSC), 20 North Start Road, Eloy, AZ 85132. I understand that engaging in these activities may be hazardous, with accompanying risks of personal injury, death, and/or loss or damage to my property. In consideration of my participation in such shooting activities, I, on my own behalf and that of my heirs, successors, representatives, administrators and assigns, hereby:

1. Waive and completely release any and all, past, present or future claims, causes of action, suits, rights, damages, costs, expenses or obligations or demands of any kind whatsoever, I, or anyone on my behalf might have against LDSC, its parent, affiliates, subsidiaries or successors and their respective officers, directors, servants, employees, agents, representatives and contractors (together "LDSC"), for any loss, damage, personal injury, death and/or loss or damage to my property resulting from my participation in such shooting activities;
2. Agree to indemnify, defend and hold harmless LDSC, from all claims by any person, firm, corporation or others, for any damages, loss or claims, of any nature, arising in any way out of my participation in such shooting activities; and
3. Assume all risks associated with such activities and all responsibility for medical expenses, costs and/or other obligations and other losses or injuries to me or in which I may become involved, by reason of my participation in such shooting activities at the facility or event.

My Commitment to Firearm Safety & Handling

Anyone who uses a firearm has a responsibility to themselves and others to use the firearm safely. I acknowledge that I am familiar with the basic rules of firearm safety and that those basic rules have been explained to me. I agree to always follow all the basic rules of firearm safety during my use and handling of any firearms.

• Always keep your firearm pointed in a safe direction.
• Treat all firearms as if they were loaded.
• Keep your trigger finger outside the guard and off the trigger until you are ready to fire.
• Be certain of your target, your line of fire, and what lies beyond your target.
• Always wear appropriate eye and ear protection when shooting and maintaining your firearm.
• Always keep the gun unloaded until ready to use.

I affirm that I have read the range rules and the firearm safety rules above and fully understand that if at any time I fail to follow these safety rules, club rules, or the direction from the range officers, my privilege to these facilities may be denied. I certify that I am over eighteen (18) years of age, or have a parent/legal guardian co-signing below, and have read and understand this Waiver of Liability and have executed this instrument voluntarily on this date. You have the right to refuse this form, and LDSC has the right to refuse to let you participate if you do not sign this form.

All shooters must read, understand, and comply with the following range safety rules, policies, and procedures before shooting at the Lost Dutchman Sporting Clays range.

1. All shooters and observers must always wear eye and ear protection.
2. Treat every firearm as if it were loaded. Always point in a safe direction and be aware of downfield personnel.
3. Shotguns only. No other type of firearm is allowed to be discharged at LDSC.
4. All shotguns must have a barrel length of at least 24". Collapsible stocks are not permitted.
5. Only 7½, 8, 8½, and 9 lead shot may be used at the range. No extra-heavy loads, slugs, or buckshot is allowed.
6. Shotguns may only be loaded while in the shooting station. No walking with a loaded firearm. All actions must be kept open except when in a shooting station.
7. Shotguns may only be fired from the shoulder.
8. Shoot only at authorized clay targets.
9. Hunting is not allowed on LDSC property.
10. All range users are personally responsible for range safety, safe shooting, and must always behave safely and responsibly.
11. No more than two shells may be loaded into a shotgun at one time.
12. Only authorized LDSC range personnel are allowed down-range, beyond the shooting positions.
13. Only authorized LDSC range personnel are allowed to service trap machines and card readers.
14. Alcohol, illegal drugs, and impairing prescription drugs do not mix with shooting. Anyone appearing to be under the influence will not be permitted to shoot.
15. Unsafe conditions or practices shall be immediately reported to club management and shooting shall stop until corrected.
16. In case of inclement weather, the club manager or shooting coordinator may suspend or delay shooting for the day.
17. Any shooter whose firearm is accidentally discharged more than twice within one round shall be required to change guns.
18. Children under 18 must always be under adult supervision while inside the range perimeter.
19. In all instances not covered by these rules, range management directions and decisions are final.
20. LDSC reserves the right to modify or change rules to improve safety and shooter experience.

Safety is everyone's responsibility.

Acknowledgement of wildlife and mining activity:

1. I acknowledge that the Lost Dutchman Sporting Clays range is located on an active mining site and there may be unmarked dangers on the premises, such as old mining equipment, shafts, or other hazards.
2. I will stay on the improved trails and remain vigilant of my surroundings.
3. I will not remove or alter mining markers found on the property.
4. I will take care to protect the beautiful cactus and desert wildlife abundant on the range.
5. I will refrain from shooting any cacti, birds, animals, or reptiles.
6. I will be on the lookout for snakes, spiders, scorpions, and other dangerous animals.
